Medical Records Request

How do I submit a Request for Medical Records pertaining to the North Carolina Army National Guard?

Effective March 18, 2013, all North Carolina Army National Guard medical records requests will be requested through the G1 Medical Branch.

  1. Complete Standard Form 180 "Request Pertaining to Military Records" IAW AR 25-400-2 paragraph 10-4 to obtain your military medical records. Please allow up to 60 business days for delivery of your records. Your request will be answered in the order in which it is received.
  2. Email forms to: ng.nc.ncarng.mbx.g1-medical@army.mil fax to: 919-400-4230 or mail to:

Government Agencies Requesting Personnel Record Information (Third Parties).

 

Government agencies (such as the Federal Bureau of Investigation (FBI), the Social Security Administration (SSA), etc.) are required to submit a completed SF-86-1 "Authorization For Release of Information" in addition to SF 180 IAW AR 600-8-104.
